Summary
When Sam pulled himself and Micheal into Lucifer’s cage, he wasn’t expecting for the ground beneath his beloved twin to crumble, sending her tumbling down with them. Now trapped inside Lucifer’s Cage alongside Adam, Diana, becomes Lucifer's new plaything. The boys manage to get cas from the empty, at the cost of his grace. As hard as that is, it's even harder when cas dies only weeks later.
Talking to Rowena lets them know that he's not in hell, but Jack's awol. So it's more difficult to find out if he's in heaven. It takes them finding out that a certain archangel isn't actually dead, and a long endeavor to find him.
After all that, they realized that Castiel was not in fact in heaven, but he was in purgatory.
Dean goes on a long journey to find him, a journey that includes starting fights with the leviathan, raising all his friends from the dead. Fighting with heaven ensues, because they are under orders to keep him in purgatory. Orders from who, well obviously Jack.
