Cas swallows again, selfishly holding a bit tighter around the hunter as he mumbles, “I’m just worried about Jack,”.
Lie, Dean detects immediately. He can tell that it’s a lie. He just came in and found Cas crying with the page about himself—that Dean earnestly wrote—in the Bestiary open on the table; what the hell would Jack have to do with that?
Dean next speaks with a gentle suspicion in his voice, sounding not like he’s accusing the angel but instead like he’s opening the floor for him to tell the truth without judgment. “That’s all?” he says softly, voice comforting and familiar by Cas’s ear.
“...Yeah,” Cas says, lying again.
(TLDR: Cas dealing with his secret of the Empty deal, until he can't anymore. Lots of angsty hurt/comfort, tears, feels, and softness.)

Summary
Every time Dean looks in his rearview mirror, he sees Cas in the backseat, struggling to hold tears in his eyes. Dean takes a closer look, seeing a rather familiar expression in those eyes. When Dean was at his lowest, after being gripped tight and raised from perdition, all he could feel was guilt. He didn’t deserve to be saved. There were countless souls down there, any one of them deserved to be saved more than him. Upon another glance, Dean can see Cas’ arm hide back into his sleeve after he’s done dealing with a mosquito bite. Or, so Dean thinks.

”Really dean?” He said, with disbelief.
Dean looks at him, confused. “What?” He spoke as he blew the smoke out once again.
”The cigar.”
Dean rolls his eyes, he crushed the cigar with his fingers before throwing it on the trash bin on the side, he looked back at sam, who was still staring at him. “And?”
”And what.” Dean asks, now getting pissed off.
”Greet him.” Sam mouthed, pointing to cas with his lips, cas was busy reading a book, or is he.
Dean sighs deeply, approaching cas and kissing him in the cheek gently, now sitting beside him, he looks back at sam who was now grinning, then he looks back at the screen.
